I. Alteration Registration (12 Working Days)
1. Alteration of Name
① Requisition for alteration;
② Name alteration of affiliated company certification (original) ;
③ A Copy of Business license of affiliated company with chop;
④ Branch business license and electronic business license;
⑤ Other pertinent documents.
2. Alteration of Domicile
① Application form for alteration of the affiliated company;
② Certification of domicile usage;
A real estate certificate and its copy for self-owned house or a copy of house leasing contract and the real estate certificate of the land owner for rented house. If the above certifications are not available, other documents proving real estate ownership or domicile usage are acceptable. If the land owner is a hotel/restaurant, the business license of the hotel/restaurant is required.
③ A copy of the business license of the affiliated company (with public seal);
④ Business license and electronic business license of the branch;
⑤ Other pertinent documents.
3．Alteration of the Person in Charge of the Affiliated Company
① Application form for alteration of the affiliated company;
② Appointment letter of the person in charge of the affiliated company and his/her identification;
③ Registration form of the person in charge of the branch (original);
④ A business license copy of the affiliated company (with public seal);
⑤ Business license and electronic business license of the branch;
⑥ Other pertinent documents
4. Alteration of Business Scope of Branch
① Application form for alteration of the affiliated company;
② A business license copy of the affiliated company (with public seal);
③ Business license and the electronic business license of the affiliated company;
④ Other pertinent documents.
5. Alteration of Business Type
① Application for alteration of the affiliated company;
② Written approval of the authority (original);
③ Certification for business type alteration of the affiliated company (original);
④ A business license copy of the affiliated company (with public seal);
⑤ Business license and the electronic business license of the affiliated company;
⑥ Other pertinent documents
6. Alteration of Affiliate’s Business Duration
① Application for alteration of the affiliated company;
